In cases of a short or malformed femoral neck (e.g., after a growth disorder such as slipped capital femoral epiphysis), a relative lengthening can restore the leverage of the hip muscles, eliminate mechanical impingement, and reduce pain.
What is a relative lengthening of the femoral neck?
The indication for a relative lengthening of the femoral neck is usually Legg-Calvé-Perthes disease. In this condition, the outer part of the femur (the greater trochanter) can be positioned too high relative to the femoral head.
The problem with this is that the muscles attached to this outer part have too small a lever arm. As a result, little force can be generated, and the load on the hip joint is not optimal. This can lead to accelerated deterioration of the hip joint cartilage (osteoarthritis).
Furthermore, if a hip arthroplasty needs to be performed later, it is crucial for the outer part of the femur to be in the correct position.
What does the surgery involve?
- An incision is made on the side of the thigh.
- The outer part of the thigh bone (greater trochanter) is detached.
- This piece of bone is moved a short distance downwards, so that the hip muscles regain a good lever arm.
- The bone is reattached with 2 to 3 screws.

What can you expect after the surgery?
- The bone needs to heal and unite. This takes approximately 6 weeks.
- During these 6 weeks, you are allowed to partially weight-bear, with the aid of 2 crutches.
- After 6 weeks, the bone is sufficiently healed to start rehabilitation.
- Rehabilitation focuses on restoring muscle strength and the normal biomechanics of the hip.
